Apply for a Grant

Reporting Requirements

Grant recipients generally are required to provide one or more reports on grant outcomes by the date or dates specified in their grant award letters. Unless otherwise directed in the grant award letter, please save a copy of the following form, complete the form, then submit it to the Foundation by the date specified in your organization´s grant award letter. You may use as much space as needed or attach addenda.

If your grant award agreement included a grant objectives chart, you must submit an updated version of the chart with the report as a separate attachment.

The Foundation will acknowledge receipt of the report via email or regular mail. If additional information is needed in the course of report review, Foundation program staff will contact you.
